Assignment Overview

As the culminating assignment for this course, you will design a Health Equity Action Plan that demonstrates your professional identity as a culturally responsive, ethical, and bias-aware nurse leader. This project challenges you to apply what you have learned about diversity, ethics, bias awareness, and social determinants of health (SDOH) to create a realistic, evidence-based strategy for reducing disparities and improving outcomes in a population or practice area of your choice.

Guidelines

Select a New Focus Area

  • Choose a different health equity issue than you addressed earlier in the course.Examples include:
    • Adolescent mental-health stigma
    • Vaccine hesitancy and cultural trust
    • Access to rehabilitation for uninsured populations
    • Food insecurity among older adults
    • Bias in emergency triage or pain assessment

Describe the Problem

  • Briefly summarize:
    • Who is affected (population or community).
    • What disparities exist and the SDOH factors contributing to inequity (e.g., income, education, housing, location).
    • Evidence: Use current data or research (CDC, AHRQ, WHO, peer-reviewed journals).

Address Bias and Its Impact

  • Reflect on how explicit or implicit bias contributes to the inequity you identified.Discuss:
    • Common biases that may influence communication, assessment, or policy decisions.
    • How awareness of bias can shape your plan’s design, strategies, or education components.
    • Ways nurses can role-model bias mitigation and inclusive practice in interprofessional teams.

Develop Your Health Equity Action Plan

  • Create a short, structured plan including:
    • Goal Statement: One clear, achievable outcome to promote equity.
    • Three Action Strategies: Specific, measurable, and feasible steps nurses or teams can take.
    • Collaboration: Identify at least one interdisciplinary or community partner and describe their contribution.
    • Evaluation: Describe how you will measure progress or success (quality metrics, feedback, participation rates, etc.).

Integrate Ethical and Professional Principles

  • Link your plan to professional standards such as:
    • ANA Code of Ethics (2025)
    • AACN Essentials (2021)
    • Healthy People 2030 SDOH FrameworkExplain how your plan demonstrates integrity, advocacy, and leadershipconsistent with the BSN professional identity.

Format

  • Include a properly formatted APA 7th Edition STUDENT Title PageLinks to an external site.(includes page numbers; NO running head).
  • Your submission should be a minimum of two (2) pages no more than four (4) pages (this does NOT include the title page and reference page).
  • Must include a minimum of three (3) scholarly references to support your paper; ONE (1) from this week’s reading, and TWO (2) from professional peer-reviewed journals published within the past seven (7) years (must be credible).
  • APA 7th Edition documents should be double-spaced, including the reference page. 
  • Must use Times New Roman, Arial, or Calibri 12-pt font with 1-inch margins
