M3 W4; Case Study Plan of Care

This assignment challenges you, as a licensed RN, to apply professional nursing judgment, cultural competence, and evidence-based reasoning to real-world case study scenarios. Each scenario presents a patient and/or community situation with proposed interventions or remedies. You will critically evaluate these scenarios, agree or disagree with the remedy, and design an inclusive plan of care that addresses health equity, social determinants of health (SDOH), and health disparities. 

You will:

  1. Critically analyze one published case study from Chapter 15 of the Rose text, and
  2. Create your own case study that reflects a health need or disparity in your own area of expertise or community/population.
  3. You will then develop a health equity-oriented plan of care (remedy), integrating respect for diversity, differences, preferences, values, and social determinants of health (SDOH).

 

Guidelines

  • Part A: Textbook Case Study Analysis
    1. Select one case study from Achieving Health Equity (Ch. 15).
    2. Summarize the scenario (population, health issue, disparity, proposed remedy). (1 paragraph)
    3. State whether you agree or disagree with the proposed remedy/intervention and justify your position using at least two current (≤ 7 years) peer-reviewed nursing or health sources or national guidelines (e.g., CDC, Healthy People 2030, ANA Code of Ethics). (1 paragraph)
    4. Respond to the following Health Equity & Cultural Integration questions. (1 paragraph)
      • What social determinants of health are relevant to this case (e.g., economic stability, environment, access, discrimination)?
      • What barriers to equitable care exist?
      • How does the plan demonstrate cultural humility and respect for the patient’s values, beliefs, and lived experiences?
      • Identify a community or policy-level strategy that supports systemic equity (e.g., advocacy, interprofessional collaboration, resource linkage).
  • Part B: Student-Developed Case Study

    1. Develop a concise, original case study based on a population, patient group, or issue relevant to your nursing specialty or community/population justify your case study using at least two current (≤ 7 years) peer-reviewed nursing or health sources or national guidelines. (1 paragraph)
    2. Include basic demographics, health concerns, and 1–2 relevant SDOH factors.
    3. The case should highlight a disparity, barrier, or equity challenge you have observed.
  • Plan of Care (Remedy) DevelopmentUsing your self-developed case study, design a brief plan of care/remedy that addresses the identified health disparity using at least two current (≤ 7 years) peer-reviewed journals or health sources or national guidelines (1-2 paragraphs). Be sure to:
    • Identify key health problem(s) and underlying SDOH.
    • Propose 3–5 inclusive, evidence-based nursing interventions with concise rationales.
    • Incorporate community or policy-level strategies that promote long-term equity (e.g., partnerships, advocacy, education, access initiatives).
    • Explain how your remedy demonstrates cultural humility and respect for diversity within your population.
  • Reflection: End with a short paragraph connecting what you learned from both the textbook case and your own case study to your nursing practice. (1 paragraph)
